Help required compiling a Query

Hi,

I have a transactions table with Tran_num, card_num, transaction_date and total_points for that particular transaction.I have to write a query that gives me the summary of the number of points allocated to customers. I am looking for a result set like this..

This table will highlight how many members have collected 1 point in the last twelve months, how many have collected 2 points, etc etc...

Any help with the query is really appreciated and thanks in advance.
