Bulk Insert Help Needed

Please help.... Thanks in advance

I know all the text qualifiers can be solved in XML, here is the link...but this is not resolving the last record of my csv file, which is blank.

Using Bulk insert data is going like this

What is expected

118928  Joe               555-1313
115454                       555-1414
