combine three columns

Hello Professionals,

                              I have three columns Provider_First_Name,Provider_last_name,Provider_middle_name

Now I want to combines this three into single column to check the same table whether it has unique name records or not

I'm just wondering to know how we can do this.

Any answer is highly appreciated.
